Job summary

Hartford HealthCare Medical Group in Connecticut is seeking a registered nurse to provide skilled nursing care under provider direction across multiple clinical settings. You will triage calls, collaborate with physicians, and ensure continuity of patient care with occasional travel between Hartford and Central region offices.

The ideal candidate holds an active CT RN license and has experience in a medical/surgical office setting; proficiency with medical software is preferred.

Qualifications

• Current Connecticut RN license required.
• Experience in Medical/Surgical Office setting preferred.
• Experience with medical software (EMR) preferred.

Responsibilities

• Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings using the nursing process.
• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention promptly and professionally.
• Assists patients and family members with concerns, respects confidentiality and privacy.
• Travel between Hartford and Central region offices.
