Job summary

Hartford HealthCare Medical Group is seeking an RN to provide office-based care in collaboration with providers across Connecticut. The role includes assessing patients, planning and evaluating care, and triaging calls with prompt nursing interventions.

Requires a current Connecticut RN license and experience in a medical/surgical office setting. Familiarity with medical software and a patient-centered approach are essential to support a broad range of services and ensure continuity of care

Qualifications

• Current Connecticut RN license required.
• Previous experience in Medical/Surgical Office setting preferred.
• Experience with medical software

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of care.
• Triages telephone calls and initiates appropriate nursing intervention.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Assists patients and families with empathy; respects confidentiality and privacy.
• Neuromuscular RN; supporting Provider including IVIG, clinical triage/meds etc.
