Qualifications

• Current CT RN license and CPR certification required.
• Three years of experience in a medical practice preferred.
• Experience with medical software (EMR) desired.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and formulates nursing plan of care.
• Ensures continuity of patient care through evaluation and intervention.
• Triages telephone calls and initiates appropriate nursing interventions.
• Assists patients and families with concern and empathy; maintains confidentiality.
• Travel to other regional offices, as needed.
