Hartford HealthCare Medical Group in Connecticut seeks an RN to provide skilled nursing care in diverse clinical settings under provider direction.

The role collaborates with physicians, triages calls, assesses needs, plans and evaluates care, with focus on confidentiality and patient empathy. Requirements include CT RN license, CPR, 3 years clinical experience; cardio experience desirable; located across CT with competitive benefits.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice preferred.
• Experience with medical software.
• Cardiology experience highly desirable.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention in prompt, precise, and professional manner.
• Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy.
