Job Summary: Responsible for the direction of short-term rehab and respiratory resident care in the skilled nursing setting. Manages licensed and non-licensed clinical staff. Consults with staff, physicians, and management team on nursing problems and interpretation of facility policies to ensure resident needs are met. Maintains performance improvement activities for Nursing Services and participates in CQI activities. May assist in formulating a budget.
This position is for our subacute short-term rehabilitation unit. Must have experience with PICC lines. Trach experience preferred.
DUTIES AND RESPONSIBILITIES:
Demonstrates Competency in the Following Areas:
- Initiates and assists in the development and implementation of policies and procedures that govern the Nursing Services in collaboration with appropriate services.
- Establishes and maintains job specifications and descriptions for Nursing Services staff in conjunction with Administration. Provides education, disciplines, and evaluations of nursing staff.
- Provides 24/7 RN oversight for assigned units ensuring residents' needs are met on all shifts.
- Ensures staff demonstrate adequate documentation and are following documentation requirements, policy, and procedure.
- Represents Nursing Services on various committees, i.e., Infection Prevention and Control, and Performance Improvement.
- Makes rounds on nursing unit daily.
- Participates in the performance improvement functions by collecting and verifying data, as needed. Ensures required nursing audits are completed and delegated as needed or requested.
- Consults other departments, as appropriate, to provide for an interdisciplinary approach to the resident’s needs.
- Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families.
- Ensures licensed nurses can perform required skills per policy, assists in completion of skills competencies for assigned staff on the units. Provides teaching when needed.
- Follows direction from DON, maintains a positive professional attitude.
- Completes an accurate individualized comprehensive care plan for all assigned residents, completes a thorough RN assessment, and record review to direct care planning. Consults with resident, family, and staff regarding care preferences and care planning.
- Completes and reviews baseline care plan per DOH regulation with residents and families within 48 hours of receiving resident to unit.
- Communicates any care plan changes to staff, residents, and families to ensure care planning interventions are effective and followed.
- Attends all care plan meetings for assigned residents, reviews care plan with staff, resident, and family to ensure accuracy.
- Works closely with MDS department to ensure accurate and adequate documentation of ADL and nursing notes are in place for reimbursement.
- Ensures all risk factors and MDS triggers are identified by thorough assessment of the resident and a care plan with appropriate interventions in place for all risk factors identified.
- Monitors and reviews CMI quality measures for assigned residents, ensures triggered residents are addressed and a plan of care is in place. Educates all assigned staff regarding quality measures and oversees the overall quality of care provided on units.
- Demonstrates ability to delegate tasks appropriately, supervises LPNs and certified nursing assistants, exhibits follow-through and follow-up with delegated tasks.
- Provides RN assessments of resident required for accident/incidents, acute changes in condition, admissions, MDS, and care planning.
- Organizes workload, completing tasks in a timely manner, meets MDS deadlines.
- Provides medication to resident as needed, is able and willing to safely complete a med pass whenever needed or directed. Follows the seven (7) medication rights and reduces the potential for medication errors.
- Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
- Supports and maintains a culture of safety and quality demonstrates and participates in our Brand Integrity core values.
- Supervises licensed nurses in their daily work. Takes ownership of assigned staff outcomes, growth, and development along with employee retention rates on unit.
- Coordinates and directs resident care and education by monitoring residents’ condition and assisting licensed nurses to ensure changes in condition are identified and immediately addressed with medical, IDT, and families. Promotes the highest level of quality of life for residents ensuring a safe environment free from harm.
- Exhibits genuine interest in and appreciation of adults in the later years of life. Advocates for the residents of the facility.
- Is a motivated, independent individual who can organize workload so that all functions are completed appropriately.
Professional Requirements:
- Adheres to dress code, wears name badge, appearance is professional, neat, and clean. No long artificial nails, no large or long dangling jewelry, no large dangling earrings.
- Completes annual education requirements.
- Maintains regulatory requirements and active NYS certification.
- Maintains resident confidentiality at all times.
- Reports to work on time and as scheduled, completes work within designated time.
- Wears identification while on duty, uses computerized punch time system correctly.
- Completes education/in-services in a timely fashion. Monthly and annually and as requested.
- Attends annual review and department in-services, as scheduled.
- Attends staff meetings such as shift huddles/reports.

Regulatory Requirements:
- Graduate of an accredited school of nursing.
- Current Registered Nurse licensure in State of New York.
- Current BCLS certification.
Skills:
- Basic computer knowledge.
- Clinical knowledge of complex subacute care needs.
