Job summary

Your Health in South Carolina is seeking a Registered Nurse Case Manager to service patients across designated areas. You will travel to patients' homes and facilities, review records, and initiate telemedicine visits to coordinate comprehensive care.

The role is full-time, salary-based, with a focus on high-quality, compliant patient care, collaboration with a multidisciplinary team, and ongoing professional growth within our primary care network.

Qualifications

• Must be a Registered Nurse licensed in good standing with the state board.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings is preferred.
• Ability to travel daily to patients' homes and facilities and coordinate care.

Responsibilities

• Visit patients at home and in facilities to assess needs and initiate care plans.
• Review medical records and complete necessary consents.
• Enroll patients in Care Management if eligible and initiate a Plan of Care.
• Coordinate with care teams and providers for telemedicine when appropriate.
• Document Care Management activities accurately and timely.
