Job summary

Your Health Organization in South Carolina seeks a Registered Nurse to join our Visiting Nurse team. The Nurse Case Manager will visit patients in Bowman and surrounding areas, coordinating with providers and initiating telemedicine visits to ensure timely, quality care.

A valid RN license, good standing, and a minimum of three years of clinical experience are required; experience in community settings is preferred.

Qualifications

• Must be a licensed nurse with good standing
• Minimum of three (3) years of clinical experience
• Experience in community settings preferred

Responsibilities

• Facilitate receiving all medical records from patient’s primary provider and specialists
• Enroll patients in Care Management when eligible and initiate a Plan of Care
• Coordinate care and communicate with the care team across settings
• Document Care Management activities accurately and keep plans up to date
• Travel to patients' homes and facilities; on-call may be required
