Job summary

Gibson Area Hospital & Health Services in Gibson City, IL is seeking a Staff RN to function as caregiver and care manager, overseeing a group of patients and applying the Nursing Process with a focus on patient outcomes.

This role emphasizes assessment, data collection, care planning, documentation, patient teaching, safety, emergency response, and collaboration with the health care team. Weekend shift availability with a full-time benefits package is required.

Responsibilities

• Performs assessment and data collection focusing on the patient’s physiologic and cognitive status.
• Formulates or individualizes predefined, goal-consistent care using established nursing standards.
• Evaluates effectiveness of care by self and the health care team, and the impact of systems and environment on outcomes.
• Demonstrates competency in psychomotor skills according to written procedures.
• Identifies patient learning needs and provides instruction to meet self-care requirements.
• Performs documentation according to Division of Nursing standards and assigned areas.
• Shows awareness of patient rights and adheres to safety requirements.
• Responds effectively in emergencies following established protocols.
• Applies knowledge of care standards, protocols, drugs, lab values, and patient care resources.
• Acknowledges Illinois Practice Act in professional practice and accountability.
• Participates in staff development and peer teaching to support learning.
• Engages in ongoing learning and continuing education to advance professional development.
• Seeks validation of knowledge and decisions and requests guidance when needed.
• Maintains awareness of legal issues in professional practice and patient care to reduce risk.
• Contributes to unit and nursing goals and to development of unit standards.
• Participates in PI activities using data sources and peer review monitoring.
• Coordinates patient care among nursing staff, physicians, and support services in care manager and shift leader roles.
• Organizes and prioritizes using available resources in a time-oriented and cost-conscious manner.
• Communicates effectively using established methods and ensures timely, clear information exchange.
• Fosters working relationships with health care team members, patients, and families.
• Uses appropriate lines of authority in clinical and operational matters and seeks guidance as needed.
• Remains flexible in staffing and resolves conflicts; participates in temporary assignments as needed.
• Identifies problems using PI tools and applies sound clinical judgment for prompt action.
• Shows willingness to learn and apply theory/data to clinical settings.
