Responsibilities

• Initiate ongoing systematic assessment of physical, emotional, social, educational and functional needs of patients/families.
• Interpret assessment data to develop a nursing care plan with problems, diagnoses, orders, interventions and discharge planning.
• Collaborate with physician and preceptor as part of an interdisciplinary rehab team.
• Ensure medical records are complete, factual and timely.
