Live Well Homecare is seeking a Nurse Navigator to lead direct patient care teams, assess needs, and make care recommendations. The role emphasizes safe, timely, high-quality services through coordination with bedside nurses and discharge planning.

The Nurse Navigator will oversee pre-op education, daily rounding, discharge disposition, and follow-up care, promoting safety and efficiency within the care model.

Qualifications

• Graduate of an approved nursing program.
• Current RN license in the practice state.
• 2-3 years of relevant experience.
• Certifications: BLS required; ACLS preferred.
• Physical ability to lift, stand, walk, and perform various physical tasks.

Responsabilités

• Collaborate with bedside nurses to plan and manage patient care.
