Qualifications

• Bachelor’s degree in nursing or related field.
• Current RN license in the state of practice.
• BLS certification.
• Approximately 5 years of clinical experience.
• Strong organizational and communication skills.
• Experience in care coordination and primary care helpful.

Responsibilities

• Assess and coordinate patient care plans across teams.
• Evaluate medical necessity and coordinate tests and referrals.
• Assist patients and families through diagnostic services and care.
• Educate patients and families about care processes and resources.
• Participate in process improvement and team development.
• Build collaborative relationships with physicians, payers and care teams.
