Job summary

Corewell Health in Grosse Pointe is seeking a Nurse Navigator to assess patient needs and develop a care plan across Breast Care and related services. You will educate patients and families, coordinate with physicians and care teams, and help improve flow, reduce barriers, and ensure a seamless patient experience from admission through discharge.

Responsibilities

  • Determine appointment scheduling by assessing patient pain, severity of injury, and schedule with appropriate physician group.
  • Coordinate the care of patients in the assigned service line.
  • Ask appropriate clinical questions to determine actions.
  • Improve patient care and flow, eliminate barriers, and connect patient/family with resources.
  • Access network clinical information systems to gather and verify patient demographic and clinical information.
  • Collaborate with care providers to assist in delivering quality, cost-effective patient care.
  • Provide individualized support, education and information for seamless care.
  • Assist in identifying issues related to transitions of care and round on patients as appropriate.
  • Manage continuum of care from client facilities, giving feedback to and from Beaumont Health.
  • Serve as a liaison between patient/family and hospitals/physician office professionals.
  • Maintain understanding of services provided by Beaumont Health and associated providers.
  • Responsible for patients in inpatient and outpatient settings.

Job description

Clinical experience in Breast Healthcare, Women's health, or oncology would be highly preferred.

Scope of work

The Nurse Navigator will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as a leader and resource for the health care team. Integrates new knowledge and evidence-based research into clinical practice. This position reports to the Clinical Nurse Manager and/or the Director of Nursing.

  • Priorities include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services.
  • Access to multiple network clinical information systems to gather and verify patient demographic and clinical information.
  • Collaborate with all care providers to assist in the provision of quality, cost effective patient care.
  • Provide individualized support, education and information, helping to make care seamless, continuous and comprehensive.
  • Assist in identifying and documenting issues related to transitions of care and will round on patients as appropriate while monitoring their healthcare experience to ensure all expectations are being met.
  • Manage continuum of care from client facilities, giving feedback to and from Beaumont Health.
  • Serve as a liaison between the patient and their family and the various hospitals and physician office professionals with whom they interact.
  • Maintain a full understanding of the services provided by Beaumont Health hospitals, ordering physicians, referral centers and other organizations in the community.
  • Responsible for patients within the inpatient and outpatient setting.
Qualifications
  • Required Bachelor's Degree in Nursing
  • 3 years of relevant experience nursing experience Required
  • Registered Nurse (RN) - State of Michigan Upon Hire required
  • Basic Life Support (BLS) - AHA American Heart Association Upon Hire required Or
  • Basic Life Support (BLS) - ARC American Red Cross Upon Hire required
  • At least one License and/or Certification in area of specialty - UNKNOWN Unknown preferred
