Fairview Health Services seeks a Nurse Navigator to provide care navigation, education, and coordination for patients after referral to a clinic. The role works with providers and scheduling to meet patient needs and communicates with referring providers and care teams from referral through initial consultation.
Ideal candidates have RN licensure in Minnesota, a nursing degree (associates or bachelor's), and 3 years of relevant experience.
Provides care navigation, communication and education to patients and family members through triaging of patients after a referral to clinic is placed. Works closely with providers and scheduling team to best meet the needs of patients. Corresponds with referring providers, internal and external as needed. Open lines of communication between RN navigator and patient from referral until initial consultation with scheduling team. They anticipate patient needs and take appropriate courses of action using approved guidelines. Excellent problem solving and communication skills. Helps patient and care team navigate additional work up or testing needed prior to consultation.

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Hiring at the maximum of the range is not typical. If your role is eligible for a sign-on bonus, the bonus program that is approved and in place at the time of offer, is what will be honored.
