Job summary

Medical City Green Oaks Hospital is seeking an RN to serve as Mental Health Intake Specialist. The role focuses on onboarding patients admitted to inpatient units and supporting their discharge planning.

The RN will assess, plan, implement, and document care, coordinating with the care team to ensure safe, effective, and person-centered nursing care. Key duties include medication administration, patient education, and collaboration with physicians and other departments to optimize outcomes.

Qualifications

• RN license required.
• Associate Degree in Nursing required.
• BLS and CPI within 30 days of start date.

Responsibilities

• Coordinate care with physicians and care teams.
• Collaborate with units to deliver optimal patient care.
• Educate patients while administering medications.
• Scan medications and patients using the eMAR system.
• Follow guidelines for medication reconciliation at admission, transfer, and discharge.
• Deliver appropriate behavioral interventions within unit milieu.
• Maintain appropriate boundaries and demonstrate least restrictive interventions.
• Assess patient response and document interventions and outcomes.
• Explain diagnosis, treatment, and home care to patients and families.
