Job summary

Northwell Health Med Practice CT is seeking a Patient Care RN for the Float Team to support clinics across CT areas Wilton, New Canaan, Norwalk, Westport, Ridgefield. This part-time role offers 24 hours/week with 8-hour shifts Monday–Friday between 8:00am and 5:00pm.

Summary: To assist the clinician in providing medical care, triage by phone or in person, obtain patient histories, deliver direct care such as vaccines and wound care, coordinate with physicians, and ensure compliance with quality

Qualifications

• Active RN license valid for CT.
• BLS certification obtained within 30 days of hire.
• Experience in medical practice or ambulatory care preferred.

Responsibilities

• Triages and assesses patient conditions via phone or in-person visits.
• Provides direct patient care including wound care, vaccinations and procedures as directed.
• Obtains patient history and medical information.
• Communicates with physicians and care team about patient status.
• Prepares materials, supplies, and setup for patient exams.
• Assists clinicians to deliver care and support the patient experience.
• Complies with all practice policies and quality standards.
