Qualifications

• Assists with patient assessment, care planning and education.
• Maintains accurate documentation and uses MAPS/MCIR/EMR per policy.
• RN with current licensure and BLS certification required; Associate degree with BSN preferred.

Responsibilities

• Triage and coordinate patient care with providers and teams.
• Perform routine assessments, record vital signs and update patient records.
• Coordinate care with family and disease management plans.
• Assist with examinations and procedures as directed by the provider.
• Prepare and maintain patient areas and equipment; ensure sterile technique.
