Qualifications

• Diploma or Associate’s Degree in Nursing; RN license in New York State.
• Basic Life Support (BLS) certification.

Responsibilities

• Provides high-quality, patient-centered, clinical care.
• Performs initial and ongoing assessment of the patient with relevant data.
• Communicates and documents assessment with the interdisciplinary team.
• Utilizes resources to meet patient and family needs.
• Develops and documents individualized plan of care and services.
• Follows safety, medication administration and documentation policies.
• Revises plan of care based on evaluations and updates documentation.
