Job summary

SnapCare in Santa Fe, NM is seeking a licensed RN for Med/Surg Rehab on a 7:00 AM - 7:00 PM day shift. This full-time role includes rotating weekend and on-call duties, with a 1:5-6 nurse-to-patient ratio and a focus on patient-centered care.

The RN will float to all Med/Surg departments, perform initial assessments, develop care plans, participate in discharge planning, and provide patient education within EPIC EMR workflows. NM or Compact license and BLS are required. Dress code: grey scrubs.

Responsibilities

• Obtain and record initial patient assessments and establish priorities according to patient age, symptoms, and psychosocial needs.

• Develop and implement measurable individualized plans of care within established nursing standards.
