Job Summary

Delivers coordinated nursing care for a patient or an assigned group of patients according to established standards of care and the nursing process. Qualifications

Education and Work Experience

• Collaborates with patients, families, and healthcare providers to provide care in a safe, humane environment. Provides learning opportunities and health information to patients, families, and the treatment team.
• Participates in discharge planning to ensure continuity of care.
• Delegates and coordinates duties of the healthcare team members.
• Performs other job-related duties as assigned.
