Job summary

The Forum at Rancho San Antonio is seeking a dedicated MDS Nurse Coordinator. This RN-led role oversees the MDS, care plans and medical records for residents, ensuring accuracy, timely submissions, and compliance with HCFA and Medicare guidelines.

You will collaborate with care teams, participate in care plan conferences, and step in as staff nurse during staffing needs. Competitive benefits and a strong mission drive the workplace.

Qualifications

• Must hold a valid RN license issued by California.
• Minimum of 3 years nursing experience in skilled nursing preferred.
• Current CPR certification.
• In-depth experience and knowledge working with MDS software.
• Experience with MDS transmittal process and understanding of regulations impacting the MDS.
• Supervisory experience preferred.

Responsibilities

• Coordinates, conducts, and completes the MDS, resident care plans and conferences on the computerized system.
• Assures accuracy and completeness of entire medical record.
• Assures MDSs are completed within HCFA and Medicare guidelines.
• Ensures MDS electronic submission occurs per State regulations.
• Participates in family/resident care plan conferences.
• Maintains effective communication with other shifts and departments regarding medical records.
• Serves as a staff nurse when staffing levels require.
• Responds to emergency calls as necessary.
• Notifies physician and family of changes in resident condition.
• Encourages rehabilitative techniques to restore independence.
• Assists DON in developing medical records policies and procedures.
• Attends in-service training and education sessions.
• Performs duties assigned by supervisor.

The MDS Nurse is an RN who is responsible for the overall supervision, coordination,
maintenance and timely completion of the MDS, care plans and medical records for residents.

Salary Range: $88,733- $121,930

Employment Status: Full Time( Must be available to work in the weekends and holidays)

Here are a few of the daily responsibilities of a MDS Coordinator:

• Coordinates, conducts, and completes the MDS, resident care plans and conferences on the computerized system.
• Assures accuracy and completeness of entire medical record.
• Assures and monitors that MDSs are completed within HCFA and Medicare guidelines.
• Ensures MDS electronic submission occurs per State regulations.
• Participates in family/resident care plan conferences.
• Assures effective communication with other shifts and departments as it relates to medical record documentation.
• Serves as a staff nurse when staffing levels are necessitated in order to meet the needs of the residents.
• Responds to emergency calls as necessary.
• Assures timeliness of physician’s visits.
• Notifies physician and family of changes in resident’s condition.
• Encourages rehabilitative techniques in order to restore resident’s independence.
• Assists DON in developing and implementing medical records policies and procedures.
• Attends in-service training and education sessions, as assigned.
• Performs specific work duties and responsibilities assigned by supervisor.

Here are a few of the qualifications we need you to have:
