Job summary

Martha & Mary Health Services is seeking a dedicated Registered Nurse (MDS) to join our team in Poulsbo, WA. This role focuses on timely and accurate completion of Minimum Data Sets (MDS) for residents, working closely with the IDT to ensure regulatory compliance and quality care.

The RN MDS nurse will coordinate RAI processes, maintain precise MDS documentation, and support on-call leadership and staff training.

Qualifications

• Current RN license in Washington state.
• MDS 3.0 experience required.
• Skilled Nursing experience required.

Responsibilities

• Complete and coordinate the RAI process complying with Federal and State regulations.
• Monitor MDS and care plan documentation in medical records to support MDS coding.
• Maintain MDS data entry and status in computerized systems according to guidelines.
• Support MDS scheduling and tracking across the IDT and other meetings.
