Atrium Centers Plainwell Pines Nursing & Rehabilitation Community is seeking a full-time MDS Coordinator to drive accurate clinical documentation, regulatory compliance, and appropriate reimbursement. This role supports resident-centered care in a smaller, team-oriented facility and offers dedicated training and growth opportunities.

Ideal candidates are RNs or LPNs transitioning into MDS with strong communication, organization, and reliability.

Qualifications

• Must hold active RN license or be enrolled LPN in RN pathway.
• Long-term care experience preferred.
• MDS Coordinator experience preferred, but training available.

Responsibilities

• Coordinate and complete MDS assessments from start to finish and submit on time.
• Manage daily/weekly MDS schedules (ARDs) and due dates.
• Review charts and docs to reflect full clinical picture.
• Collaborate with nursing, therapy, dietary, and social services for accurate information.
• Lead or participate in care plan meetings to align with resident goals.
• Identify missing documentation and resolve before submission.
• Monitor Case Mix (CMI) and support appropriate reimbursement.
• Assist with audits, surveys, and quality reviews with organized documentation.
• Educate staff on documentation best practices.
• Serve as primary contact for MDS, care planning, and regulatory compliance.

What You’ll Do

• Coordinate and complete MDS assessments from start to finish, making sure everything is accurate and submitted on time

• Manage daily and weekly MDS schedules (ARDs), keeping track of due dates and making sure nothing falls behind

• Review resident charts, therapy notes, and nursing documentation to ensure assessments reflect the full clinical picture

• Work closely with nursing, therapy, dietary, and social services to gather the right information and keep documentation aligned

• Lead or participate in care plan meetings, helping ensure each resident’s plan reflects their needs and goals

• Identify missing or inconsistent documentation and follow up with the team to resolve it before submission

• Monitor Case Mix (CMI) and support accurate reimbursement by ensuring documentation supports the services being provided

• Assist with audits, surveys, and quality reviews by maintaining organized, compliant, and survey-ready documentation

• Help educate and support staff on documentation best practices to improve accuracy and outcomes

• Serve as a key point of contact for anything related to MDS, care planning, and regulatory compliance

What You Bring

• Current, active Registered Nurse (RN) state license OR LPN actively enrolled in an RN program

• Long-term care experience preferred

• Prior MDS Coordinator experience preferred, but training is available for the right candidate

• Strong written and verbal communication skills

• Detail-oriented, organized, and deadline-driven

• Positive, dependable, and team-focused attitude
