Nashua Post Acute and Healthcare Center is seeking a full-time MDS Coordinator to ensure timely, accurate MDS assessments for resident care planning and reimbursement. You will supervise data entry, analyze data, and support regulatory reviews in a collaborative team environment.

The role requires an active RN license and at least 3 years of long-term care experience, including MDS/RAI familiarity, with strong attention to detail and knowledge of Medicare/Medicaid requirements.

Qualifications

• RN/BSN/LPN nursing degree required.
• Minimum of 3 years of clinical long-term care experience.
• Prior MDS/RAI experience required and knowledge of regulations.

Responsibilities
