Avalon Care Center Honolulu is seeking an experienced MDS Coordinator (RN) to assess residents, complete MDS documentation, and collaborate with the care team to develop individualized plans.

This on-site role offers a competitive hourly wage of $48.00–$55.50, strong benefits, and opportunities to impact quality outcomes, reimbursement accuracy, and resident care at a trusted Honolulu facility.

Qualifications

• Current Hawaii RN license.
• Strong knowledge of the RAI process and case management.
• Strong attention to detail and organizational abilities.
• Ability to meet all employment screening requirements.

Responsibilities

• Complete resident assessments and MDS documentation
• Partner with the care team to develop resident care plans
• Ensure assessments are accurate, timely, and compliant
• Support Medicare and Managed Care processes
• Help drive quality outcomes and reimbursement accuracy
