• Monitor and report changes in resident condition, and facilitate communication among healthcare providers
• Participate in quality improvement initiatives and contribute to the development of process improvements

About the Role

As a RN MDS Coordinator, you will be responsible for coordinating the care of our residents through the MDS (Minimum Data Set) process. This critical role requires strong clinical knowledge, exceptional organizational skills, and the ability to communicate effectively with healthcare professionals. You will work closely with our interdisciplinary team to ensure that our residents receive comprehensive care that meets their unique needs and goals.
