Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking a qualified MDS Coordinator RN to join our team. The ideal candidate will oversee MDS, CAAs, and care plans, ensuring compliance with federal and state regulations while coordinating with physicians, families, and payers.

Responsibilities include timely submission of MDS data, working with IDT members, analyzing QI/QM data, and educating staff about RAI processes. RN licensure and LTC experience required; salary up to $140,000.

Qualifications

• Must be a Registered Nurse with active license.
• Minimum two years of clinical experience in a healthcare setting.
• Minimum one year of experience in a long-term care setting.
• Prior experience as an MDS coordination is preferred.
• Training program available for RN candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate and thorough completion of the MDS, CAAs and Care Plans per regulations and guidelines.
• Acts as in-house Case Manager coordinating services with physicians, families, third party payers, and facility staff.
• Proactively communicates with Administration to identify regulatory risk and improve MDS processes.
• Captures the RUG score reflecting care and services provided.
• Ensures timely electronic submission of all MDS to the state data base.
