Job summary

Prestige Care Center of Nebraska City is seeking an experienced MDS Coordinator to lead MDS processes, care planning, and interdisciplinary collaboration. You will ensure accurate RAIs, supervise nursing staff, and integrate infection prevention across the facility.

The role emphasizes policy compliance, scheduling efficiency, and ongoing staff training to support safe, high-quality resident care in a team-driven environment.

Qualifications

• Must possess an active RN license in Nebraska.
• 3 years of RN experience with one year in a supervisory role.
• RAC training within six months of hire; RAC certification preferred.
• Experience or training in infection control and prevention.
• Specialized infection prevention training completed.

Responsibilities

• Coordinate and complete RAIs and implement resident care plans.
• Schedule and lead interdisciplinary care plan meetings.
• Audit medical records and educate staff on documentation.
• Collaborate with health plans and physicians for approvals and coverage.
• Oversee MDS/nursing scheduling and IP coordination duties.
• Develop and maintain infection prevention and control programs.
• Lead infection prevention committee and annual risk assessments.
• Maintain monthly staff scheduling, attendance and PTO processes.
