Job summary

MB Healthcare is seeking an experienced MDS Coordinator to join our Madison, NJ facility. The on-site role focuses on coordinating and completing MDS assessments accurately and on time, ensuring regulatory compliance, and supporting reimbursement processes related to MDS documentation.

You will collaborate with nursing and interdisciplinary teams to translate resident assessments into individualized care plans, monitor changes in status, educate staff on MDS procedures, and maintain organized

Qualifications

• Current RN license in good standing with the applicable state board.
• Experience with MDS assessments and related regulatory requirements.
• Strong skills in care planning and developing individualized resident care plans.
• Knowledge of geriatric long-term and short-term care is preferred.
• Experience in skilled nursing facility operations and reimbursement processes is preferred.
• Excellent communication and documentation with interdisciplinary teams.
• Strong organization and time management in a fast-paced clinical setting.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Ensure regulatory compliance and facility policies are followed.
• Collaborate with nursing and interdisciplinary team members on care plans.
