Job summary

RAM Recruiting is seeking an experienced MDS Coordinator for a full-time role in the Far Rockaway/Queens area. The ideal candidate will be an RN with proven MDS coordination experience in a skilled nursing setting, capable of leading the RAI process and care planning with the interdisciplinary team.

Responsibilities include managing MDS assessments, PDPM reimbursement documentation, and ensuring compliance with federal and NY state regulations.

Qualifications

• Current Registered Nurse (RN) license in New York State.
• Prior experience as an MDS (Minimum Data Set) Coordinator.
• Experience in a skilled nursing facility (SNF) / long-term care (LTC) setting.

Responsibilities

• Complete and coordinate MDS (Minimum Data Set) assessments accurately and on schedule.
• Manage the RAI (Resident Assessment Instrument) process and lead care plan meetings with the interdisciplinary team.
• Ensure documentation supports accurate PDPM (Patient-Driven Payment Model) reimbursement.
• Maintain compliance with federal and New York State assessment requirements.
• Work with nursing and therapy teams to keep assessments aligned with residents' actual clinical status.
