Emerald Nursing and Rehabilitation, located in Elizabethtown, PA, seeks an RN MDS Coordinator to lead MDS assessments and care planning in a long-term care setting. You will collaborate with an interdisciplinary team to ensure accurate documentation and compliance with MDS/RAI requirements.

The role requires an active PA RN license and hands-on MDS experience, with a focus on accurate coding and optimal reimbursement.

Qualifications

• Must be a PA-licensed RN with hands-on MDS experience in long-term care.
• Strong knowledge of MDS 3.0, RAI process and care planning.
• Ability to ensure accurate documentation and regulatory compliance.

Responsibilities

• Complete and submit MDS assessments and care plans per regulatory requirements.
• Monitor, evaluate, and update resident care plans as needed.
• Collaborate with interdisciplinary team for thorough documentation.
• Assist in optimizing reimbursement through accurate clinical documentation and coding.
• Participate in QAPI initiatives and compliance activities.
• Maintain current knowledge of MDS/RAI regulations and processes.
