Qualifications

Responsibilities

• Coordinate and conduct MDS assessments with accurate documentation
• Develop and implement individualized care plans with the healthcare team
• Monitor resident progress and drive improvements
• Collaborate with physicians, therapists and staff for comprehensive care
• Maintain up‑to‑date records per regulations and policies
• Provide education and training to staff on best practices
• Participate in quality improvement initiatives and policy development
