Responsibilities

• Maintain and update schedules of MDSs that are due.
• Coordinate the work of the Interdisciplinary Team to complete the MDS.
• Enter or supervise input of MDS into computer systems.
• Act as the leader of the MDS and care plan meetings.
• Coordinate the completion of the care plan.
• Other tasks as required.
