Job summary

Diversicare in North Carolina is seeking an RN to join as MDS Coordinator/Clinical Coordinator to oversee accurate and timely MDS assessments and ensure compliance with state and federal regulations.

The role leads care coordination with interdisciplinary teams, supervises documentation, and guides the care management process across the facility while upholding Diversicare values and delivering high-quality resident care.

Qualifications

• RN license in state of employment required.
• Previous MDS/RAI experience.
• Skilled nursing/long term care regulatory knowledge preferred.
• Knowledge of PDPM, managed care reimbursement, utilization and discharge planning preferred.
• Experience leading teams and projects to achieve outcomes.
• Strong verbal, written and interpersonal skills.
• Proficient with computers/various software platforms.

Responsibilities

• Coordinate the RAI process and ensure accurate MDS assessments.
• Ensure timely MDS submissions per regulations.
• Collaborate with interdisciplinary team for thorough documentation in records.
• Maintain MDS tracking schedules and audit compliance.
• Provide education on the RAI process to staff.
• Obtain and review state/federal reports and correct as needed.
• Monitor Quality Measures and align MDS with outcomes.
• Ensure regulatory guidelines (Medicare/Medicaid) are followed.
• Communicate with patients/families about changes in condition.
• Coordinate care planning and care conferences.
• Lead daily care coordination meetings.
• Facilitate 72 Hour meetings and care management discussions.
• Complete aftercare calls and coordinate restorative nursing activities.
• Record all therapy authorizations and managed care interactions.
• Utilize PCC, SHP, and Master tools for care management.
• Facilitate patient rounds and discharge planning discussions.
• Participate in end-of-month billing review processes.
• Handle other assignments from Administrator.
