Job summary

Healthcare Nursing of Syracuse is seeking an experienced MDS Coordinator / MDS Assessor to join their clinical team in the Syracuse, NY area. The role requires MDS 3.0 expertise and an active NY RN license.

You will complete MDS assessments and care plans, coordinate monthly schedules, and ensure regulatory compliance. Strong organizational, communication, and teamwork skills are essential in a supportive clinical environment.

Qualifications

• Active New York State RN license required.
• MDS 3.0 experience is a must.
• Experience in clinical care coordination or assessment role preferred.
• Strong organizational and time management skills.
• Solid computer and documentation skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Complete accurate MDS assessments, documentation, and care plans per regulations and facility standards.
• Initiate and update individualized care plans as assigned.
• Create and distribute monthly care plan schedules in a timely manner.
• Maintain and update clinical assessments and documentation as required.
• Monitor and audit clinical records for accuracy, compliance, and timeliness.
• Communicate documentation concerns and compliance issues to nursing leadership.
• Maintain strict confidentiality of patient and organizational information.
