Job summary

Qualifications

Responsibilities

Complete accurate MDS assessments, documentation, and care plans per regulations and facility standards.
Initiate and update individualized care plans as assigned.
Create and distribute monthly care plan schedules promptly.
Maintain and update clinical assessments and documentation as required.
Monitor and audit clinical records for accuracy, compliance, and timeliness.
Communicate documentation concerns and compliance issues to nursing leadership.
Maintain strict confidentiality of patient and organizational information.
