Job Responsibilities

• Knowledge of RAI/MDS process including person-centered care planning, Medicare PDPM reimbursement, scheduling, month-end checks, appeals, and Medicare certification
• Maintain current PDPM/Medicare and Medicaid assessment schedules for coordination with therapy, dietary, social services, activities, and nursing
• Provide ongoing education and training on MDS and person-centered care planning
• Follow through with RAI process, including assessments, triggers, CAAS, and care planning
• Maintain Medicare certification/recertification documentation to meet certification requirements
• Demonstrate current nursing practices and procedures relevant to Long Term Care and regulations

Minimum Requirements

• Registered Nurse (RN) license in New Hampshire
• RAC-CT certification preferred
• At least 2 years’ experience in long-term care, including RAI process and care planning
• Strong clinical and communication skills
• Extensive knowledge of RAI process and reimbursement regulations
• Highly organized with multitasking ability
• Experience with Microsoft Office suite preferred
