Job summary

Haven of Tuscola in Tuscola, IL is seeking an RN MDS / Care Plan Coordinator to lead resident assessments and develop individualized care plans in a long-term care setting.

You will chair interdisciplinary care plan meetings, ensure accurate EMR documentation, and mentor staff on MDS processes to improve quality of life for residents. This full-time role offers a collaborative team environment and meaningful work.

Qualifications

• Active RN license in Illinois.
• Minimum 1 year of MDS experience.
• AANAC certification preferred.
• Strong knowledge of MDS 3.0, RAI process, and long-term care regulations.
• Excellent communication, leadership, and organizational skills.
• Experience with EMR systems.

Responsibilities

• Complete and coordinate MDS assessments (admission, quarterly, annual, and significant change).
• Develop and maintain individualized, resident-centered care plans.
• Lead interdisciplinary care plan meetings and drive positive outcomes.
• Ensure accurate EMR documentation and compliance with state and federal regulations.
• Provide staff education and support on MDS processes and care planning best practices.
• Collaborate with nursing, therapy, and administrative teams to optimize care delivery.
