Job summary

Haven of Tuscola is seeking a skilled RN MDS / Care Plan Coordinator to lead and manage resident assessments and individualized care planning in a long-term care setting. You will oversee MDS processes, coordinate care plans, and participate in interdisciplinary meetings.

This full-time role requires an active IL RN license, MDS experience, and strong communication and leadership skills to improve resident outcomes and compliance with regulations.

Qualifications

• Active RN license in Illinois is required.
• Minimum 1 year of MDS experience is required.
• AANAC certification is preferred.
• Strong knowledge of MDS 3.0, RAI processes, and long-term care regulations.
• Excellent communication, leadership and organizational skills.
• Experience with EMR systems.

Responsibilities

• Complete and coordinate MDS assessments (admission, quarterly, annual, and significant change).
• Develop and maintain individualized, resident-centered care plans.
• Lead interdisciplinary care plan meetings and drive positive outcomes.
• Ensure accurate EMR documentation and compliance with state and federal regulations.
• Provide staff education and support on MDS processes and care planning best practices.
• Collaborate with nursing, therapy, and administrative teams to optimize care delivery.
