REPUBLIC NURSING & REHAB in Republic, Missouri is seeking an RN MDS Coordinator to join our care team. The role focuses on completing MDS assessments and care plans, initiating care planning activities, and maintaining timely calendars.

Ideal candidates have a valid RN license and prior MDS experience in long-term care, strong organization and communication skills, and the ability to work 40 hours weekly. You will collaborate with the DON and care teams to ensure accurate documentation and

Responsibilities

• Complete accurate MDS assessments and care plans as assigned.
• Initiate care planning activities and supporting tasks.
• Create and distribute monthly care plan calendars in a timely fashion.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform the DON of persistent issues related to non-compliant documentation.
• Participate in meetings and committees (Medicare, QAPI, Weight Management, etc.).

REPUBLIC NURSING & REHAB, located in Republic, Missouri is currently seeking an RN MDS Coordinator to join our team.

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN License

• Knowledge of skilled nursing facilities survey process and regulations would be helpful.

• Must have prior MDS Experience

• Long Term Care Experience Required

• Must be able to work 40 hours a week

• Must be highly organized, professional & motivated

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker
