Pay Rate: $36–$42 per hour, depending on experience
Job Description:
Charlotte Bay Rehab and Care Center is seeking a knowledgeable and detail-oriented RN MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process. The ideal candidate will have experience with PDPM and Medicare documentation, ensuring compliance with all regulatory guidelines while supporting high-quality resident care.
Key Responsibilities:
Accurately complete and coordinate all MDS assessments in compliance with state and federal regulations.
Implement and maintain effective care plans based on assessment results.
Collaborate with interdisciplinary teams to ensure continuity and quality of care.
Monitor PDPM requirements and ensure documentation supports Medicare claims.
Review and analyze QI/QM reports and implement corrective measures as needed.
Participate in surveys, audits, and inspections as required.
Submit assessments electronically and maintain accurate records and schedules.
Educate staff on MDS/PDPM updates and best practices.
Requirements:
Active RN license in the state of Florida.
Proven experience with PDPM and Medicare documentation required.
Minimum of 2 years’ experience in a skilled nursing facility preferred.
Strong understanding of MDS 3.0, care planning, and RAI processes.
Excellent communication, leadership, and organizational skills.
