Job summary

Morristown Post Acute is seeking an RN MDS Coordinator to join our dedicated team. This role focuses on coordinating MDS processes, guiding nursing staff, and ensuring accurate EMR documentation to support high-quality resident care.

The ideal candidate will possess RN licensure, experience in post-acute settings, and strong leadership and training abilities to collaborate with interdisciplinary teams.

Qualifications

• RN licensure with relevant experience in post-acute care or long-term care.
• Strong management, organizational and interpersonal abilities.
• Proficient with EMR systems and clinical documentation.
• Ability to train nursing staff on care plans and EMR usage.

Responsibilities

• Coordinate MDS processes and timelines across care teams.
• Supervise and guide nursing staff in care planning.
• Conduct in-service trainings on care plans and EMR usage.
• Monitor resident EMR records for accuracy and completeness.
• Identify problems and communicate to the DON and staff.
• Maintain professional, courteous interactions with residents and families.

MDS Coordinator Responsibilities

• Must have working knowledge of nursing services, nursing administration, rehabiliation, general nursing, geriatric nursing, the MDS, and its computer application (EMR)
• Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans
• Ability to conduct in-service training of nursing staff on care plans, documentation, and EMR application.
• Ability to monitor resident EMR records for consistency and content.
• Ability to identify problems and to communicate those problems to the DON and staff concerned
• Be personable with residents, families, and staff in a professional and cooperative manner
