Job summary

Morristown Post Acute is seeking an experienced MDS Coordinator to join our dedicated team in Morristown, NJ. The role demands strong MDS knowledge, nursing administration background, and expertise with EMR to manage care plans and documentation.

You will train staff, monitor resident records, and communicate issues to the DON, while delivering compassionate, resident-centered care in a friendly environment.

Qualifications

• Must have working knowledge of nursing services, nursing administration, rehabilitation, general nursing and geriatric care.
• Must be able to initiate, complete and update care plans with strong organizational skills.
• Ability to conduct in-service training of nursing staff on care plans and EMR use.
• Ability to monitor resident EMR records for consistency and content.
• Identify problems and communicate them to the DON and staff.
• Be personable with residents, families, and staff in a professional manner.

Responsibilities

• Must have working knowledge of MDS and its computer application (EMR).
• Manage and coordinate care plans with strong planning and organizational ability.
• Conduct in-service training for nursing staff on care plans and EMR.
• Monitor EMR records for accuracy and consistency.
• Communicate problems to the DON and relevant staff.
• Maintain a professional, friendly attitude with residents and families.

MDS Coordinator Responsibilities

• Must have working knowledge of nursing services, nursing administration, rehabi litation, general nursing, geriatric nursing, the MDS, and its computer application (EMR)
• Must have good managerial, organizational, plannings, computer, and interpersonal ability to initiate, complete and update care plans.
• Ability to conduct in-service training of nursing staff on care plans, documentation, and EMR application.
