Livingston Hills Rehabilitation & NCC seeks an experienced RN MDS Coordinator to join our team in a 120-bed SNF in Columbia County, NY. The role focuses on timely, accurate assessments and coordinating interdisciplinary care plans with families and care teams.

The successful candidate will understand CMI, maximize CMI, and have knowledge of Medicaid and Medicare, with a commitment to quality, compliance, and ongoing professional growth.

Qualifications

• Current license as a Registered Nurse (RN) in NY.
• Experience as an MDS.
• Must understand CMI and maximize CMI.
• Knowledge of Medicaid and Medicare.

Responsibilities

• Assuring timely and accurate assessments of interdisciplinary care plans.
• Assist in identifying resident needs; communicate needs & expectations to families.
• Information from assessments helps caretakers formulate care plans with input from social services, dietitians, rehab specialists & medical staff; MDS coordinators monitor care plans for effectiveness & compliance.

• Assuring timely and accurate assessments of interdisciplinary care plans.
• Assist in identifying resident needs; communicating specific care needs & expectations to families.
• Information from assessments help caretakers formulate care plans, (in addition to support from social services, dieticians, rehab specialists & medical staff). MDS coordinators implement and monitor these care plans to ensure effectiveness & compliance.

Requirements:

• Current license as a Registered Nurse (RN) in the state of NY.
• Experience as an MDS.
