Responsibilities include timely assessments of interdisciplinary care plans and ensuring compliance, with collaboration from social services, dietitians, rehab specialists and medical staff. NY RN licensure and MDS experience required.

Qualifikationen

• Current license as an RN in NY.
• Experience as an MDS coordinator.
• Must understand CMI and maximize CMI.
• Knowledge of Medicaid and Medicare.

• Assuring timely and accurate assessments of interdisciplinary care plans.
• Assist in identifying resident needs; communicate care needs & expectations to families.
• MDS coordinators implement and monitor care plans to ensure effectiveness & compliance.
