Job summary

Allure HCS is seeking a seasoned MDS/Care Plan Coordinator to ensure accurate and timely completion of Medicare/Medicaid case-mix documents, supporting appropriate reimbursement within the facility. The role involves coordinating interdisciplinary teams, conducting regular MDS reviews, scheduling PPS/MDS assessments, and maintaining compliant documentation across regulatory standards.

Strong RN licensure with Long Term Care experience and proficiency in English are required; experience with MDS

Qualifications

• Registered Nurse licensed in the state.
• Long Term Care experience preferred.
• Ability to read, write, speak and understand English.

Responsibilities

• Assesses health status and level of care of new admissions.
• Ensures timely completion of all MDS Assessments for PPS Medicare.
• Communicates level of care to all disciplines.
• Coordinates interdisciplinary participation for MDS completion within regulatory timeframes.
• Maintains accurate MDS assessment schedules across resident stays.
• Responsible for data entry and electronic submission of MDS assessments.
• Verifies submissions and maintains records.
• Schedules and conducts resident care conferences and ensures MDS reviews are complete.
• Assists in formulating and revising care plans with emphasis on appropriate nursing interventions.
