Job summary

Momentum at South Bay for Nursing and Rehab seeks a qualified RN to join as an RN MDS Assessor. A NYS RN license with 3 years' experience is required, plus solid computer skills and knowledge of MDS/PPS.

Excellent benefits include health, dental, vision insurance, PTO, and 401k matching. Schedule is M-F mornings, with weekends as needed; salary is $103,000 annually and full-time employment.

Qualifications

• Excellent communication skills a must, with the ability to follow oral and written instructions.
• Solid computer skills required, SIGMACARE experience preferred.
• Has a working knowledge of MDS and PPS process and able to meet deadlines.

Responsibilities

• Coordinate MDS PPS processes and assess residents for MDS eligibility.
• Ensure accurate and timely documentation to support MDS data.
• Work independently and as part of a team to meet deadlines.
• Detailed, organized approach to scheduling and workload management.
