Job summary

Park Place Health and Wellness Center in St. John, IN is seeking an RN Manager for Long Term Care to lead nursing operations across Assisted Living, Memory Care and Long-Term Care.

This full-time role reports to the Director of Nursing and requires strong leadership and regulatory compliance. Responsibilities include scheduling, audits, infection control, participating in committees, onboarding new hires, and ensuring quality care.

Qualifications

• Must be a Registered Nurse (RN) in Indiana.
• Prior experience as a manager/supervisor or scheduling in skilled nursing/long-term care is a plus.
• Pass a physical, drug screen, 50-pound lift test, fingerprint for background, and complete orientation.

Responsibilities

• Assist LTC Clinical Operations in Assisted Living, Memory Care and Long-Term Care.
• Ensure daily operations comply with applicable standards and regulations.
• Assist QA&A with plans to correct deficiencies.
• Complete audits and infection control program in the building.
• Schedule daily rounds to observe clients and nursing staff.
• Make reports to the Director of Nursing as required, and participate in surveys.
• Attend facility committees as required (Infectious Control, Policy Advisory, etc.).
• Maintain master forms on computer (schedule, attendance, etc.).
• Assist with onboarding and training of new hires.
• Take after-hours calls and work on weekends/on-call as needed.

Summary of essential job functions:

• Assist LTC Clinical Operations in Assisted Living, Memory Care and Long-Term Care.
• Ensure the daily operations are in accordance with current federal, state and local standards, guidelines and regulations.
• Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies.
• Complete necessary audits and Infection Control program in the building.
• Schedule daily rounds to observe clients and nursing staff, ensuring that quality care is provided to clients.
• Make written and oral reports/recommendations to the DON as necessary/required, concerning nursing operations.
• Participate in PLS surveys made by authorized government agencies.
• Attend various committees of facility (i.e., Infectious Control, Policy Advisory, Pharmaceutical, Budget, Quality Assessment and Assurance, etc.) as required, and as appointed by the administrator.
• Maintains master forms on computer for: Floor schedule, calendar attendance, trade slips, call-in slips, leave early slips, payroll deduction sheet, new schedule sheet, etc.
• Assist with on-boarding and training of new hires.
• Take and make phone calls after hours.
• Work the floor when necessary; and every other weekend on call. Some holidays on call.
• Reports to Director of Nursing.

Requirements

• Must be a Registered Nurse (RN) in the State of Indiana.
• Prior experience as a manager/supervisor or scheduling with skilled nursing/long-term care a plus.
• Pass a physical, drug screen, 50-pound lift test, fingerprint for criminal background and complete the orientation program.
