Job summary

The Paramount at Somers Rehabilitation and Nursing Center is seeking a qualified Registered Nurse to join our skilled and long‑term care teams. You will provide direct patient care, monitor progress, and collaborate with a multidisciplinary team to restore residents to their highest potential.

Qualifications

• New York State RN license required.
• Experience in SNF or long-term care preferred.
• Excellent communication and patient-care skills.

Responsibilities

• Provide resident care in SNF and long-term care units.

• Provide resident and family education.
• Coordinate with physicians, NPs, families and other health care professionals.

Job Description:

• Provide care for residents of our Skilled Unit (SNF) and Long Term Care Unit. Duties Include;
• Medication Administration, including IVs.
• Taking and recording vital signs.
• Providing resident care and treatments with monitoring of outcome.
• Provide resident and family education.
• Communication with physicians, NPs, families and other health care professionals.
• Supervising CNAs

QUALIFICATIONS/REQUIREMENTS:

• Current New York State License- RN
