Qualifications

• Licensed Registered Nurse in Florida with active license.
• Minimum three years case management experience and two years leadership.
• Bachelor’s degree in Nursing or higher preferred.

Responsibilities

• Lead case management and discharge planning with 24/7 accountability.
• Develop and implement goals, plans, and standards per regulatory needs.
• Oversee staffing, performance reviews, and education initiatives.
• Collaborate with multi‑disciplinary teams to improve patient flow.
• Monitor quality, compliance, and performance improvement activities.
