Job Summary
The Case Management Manager works with the interdisciplinary care team to facilitate effective patient care during hospitalization, using a proactive approach.
- Facilitate effective and efficient patient care during hospital stay, including face-to-face assessments for discharge needs considering the patient's needs, functional capacities, resources, and support systems.
- Conduct follow-up visits and phone calls with patients and/or families as needed.
- Complete accurate and timely utilization reviews of patient charts to ensure appropriate hospital resource utilization.
- Provide clinical information to third-party payers to support patient admission and/or continued stay.
- Communicate actively with physicians regarding length of stay, admission, treatment criteria, and cost-effective service delivery.
- Perform comprehensive assessments considering patient needs, capacities, resources, and support systems.
- Collaborate on discharge plans, including follow-up services like equipment, home health, or placement.
- Advocate for the patient’s right to self-determination.
- Serve as a liaison among patients, families, staff, physicians, and community providers.
- Provide community information and referrals.
Qualifications Needed
- Registered Nurse
- Bachelor's Degree REQUIRED
- Minimum 2+ years supervisory or management experience in a case management unit within an acute care facility REQUIRED
